Bright and Associates offers Care Management and Behavioral Health Home (BHH) services to help clients access the resources, services, and supports they need to live well.

Who Is Eligible?

BHH services are available to adults enrolled in Minnesota Medicaid who have a serious mental illness diagnosis and meet specific functional criteria. Our team will conduct an intake assessment to determine eligibility and connect eligible clients with the appropriate tier of services.
